How To Purchase The Medical Board's Database
The Composite State Board of Medical Examiners has available for purchase updated lists for physicians (Medical Doctors (M.D.) and Doctors of Osteopathy (D.O.), physician's assistants, respiratory care professionals, acupuncturists, orthotists, prosthetists, auricular detoxification specialists, and resident training physicians.
The database disk will include both a text (tab delimited) and Excel file. The included fields are: Name, address (street, city, state, zip code), license or certificate number, license or certificate original issuance date, license or certificate status (active, inactive, lapsed, revoked, etc.), specialty code, public board orders flag ("Yes" or "No"), and date of board order. No other information is available.
All database orders must be accompanied with full payment of the appropriate fee(s) on the order form. Requests received without full payment will be returned unprocessed. To order a database, you may put the request in writing to:
